Approved City of Victoria Backflow Assembly Test Form
All backflow tests must be completed on the TCEQ approved City of Victoria Backflow Assembly Test Form.
The signed and dated original form must be submitted to the Public Works Department. All completed backflow test forms must be submitted within ten (10) days of testing. Please ensure all applicable criteria on the form is properly marked to prevent the test from being out of compliance.
Testers should submit gauge calibration records on an annual basis.
